Indispensable Fernandes was unfortunate to strike the post from outside the box but was alert and interchanged well with Fred on the left-flank.
His cross was not dealt with by Fulham goalkeeper Alphonse Areola and Cavani pounced from six yards.
Ademola Lookman had given the home side the lead inside give minutes after being played onside by Aaron Wan-Bissaka.
